You can pre-pay for your appointments (see our package deals), or pay at the time of your visit and we will do the rest of the paperwork for you.

 

Bring your health insurance information to your next appointment, or complete and bring in the attached form, and we can get started on submitting your claims for you.

 

You should see your reimbursement from your insurance company in your account within a couple of days.
